As a new Miracle Travel Independent Representative Applicant, I understand and agree that: I am of legal age to enter into this agreement in the state in which I reside. This agreement becomes effective when received and accepted by Miracle Travel at its home office. Upon acceptance of this application I will become an Independent Representative of Miracle Travel, have the right to sell Miracle Travel products and services, and be eligible to receive compensation as defined in the policies and procedures. I shall be an Independent Contractor responsible for my own business activities and not an employee or legal representative of Miracle Travel. I shall pay all applicable Federal and State income taxes, self-employment taxes, local taxes, and/or local license fees that may become due as a result of my activities under this agreement. I understand and agree that request for taxpayer identification number form W-9 must be on file at Miracle Travel's home office to receive any compensation. I may terminate this agreement at any time by submitting a written resignation to Miracle Travel. I understand and agree that Miracle Travel may terminate this agreement in accordance with the Policies and Procedures if I violate the terms thereof. The term of this agreement is one year. It must be renewed annually, and is subject to acceptance by Miracle Travel. The renewal process and fee will be set out in Miracle Travel's Policies and Procedures. I understand and agree that Miracle Travel may make changes in the Policies and Procedures, Compensation Plan, Prices, etc. at any time. I understand and agree that this agreement may not be transferred without the prior written consent of Miracle Travel. I shall conduct myself in a legal, professional, and ethical manner at all times. I shall not make any statements or representations whatsoever regarding Miracle Travel or its products and services that are not contained in official literature or website. I shall not use Miracle Travel trademarks, trade style, or trade name in any form other than provided or approved in writing. I shall not use any written, recorded, or other materials not provided or approved in writing by Miracle Travel. I have no authority to bind Miracle Travel to any obligation.
